docs: P1-22~P1-24 补充 — Dispatch 桥接、指令对齐、Merge 同步

新增设计章节 3.2.11b~3.2.11c：
- Dispatch → Worker 启动桥接（spawn_workers + 指令操作化 + 工具白名单对齐）
- Merge → TaskGraph 状态同步（merge 后更新 task-graph.json 节点 status）

新增缺陷记录 P1-22/P1-23/P1-24 + 实施任务 T-1.21/T-1.22 + KPI 指标
